If you are injured and have received a prescription for physical therapy from your doctor, you are eligible for insurance coverage of physical therapy. As a courtesy to you, we will submit all charges to your insurance company. Keep in mind that many insurance plans require the patient to satisfy a deductible, co-insurance, and/or co-pay as part of coverage.

We accept Medicare and are affiliated with a number of HMO and PPO plans. In most cases, we will accept in-network benefits for other PPO and POS plans. Simply call our insurance coordinator at (847) 437- 7070 for details. We will be happy to verify insurance coverage for your treatment at Midwest Physical Therapy prior to your first appointment. We will also let you know if your particular insurance plan requires you to obtain a referral or pre-authorization for particular services.

If you are not injured and simply desire to receive our injury prevention services we will arrange for payment on a cash basis.
